Most people know that smoking can endanger the health of himself and others, but not a few people who remained stubborn. Here it is the reasons people stay in recalcitrant smokers.

"From upstream to downstream Rooms all contain toxins, and in the long term the resulting diseases can really spend money," said Dr. Sally Aman Nasution, SpPD-KKV, FINASIM in a press conference about smoking in the Secretariat PAPDI PAPDI PB, Cikini , Jakarta, Friday (4/2/2011).

Dr Sally said there are cases of a man an 80-year-old son brought a heart disease. The man was known as a smoker and he often dodged that he was just a heart disease and smoking did not feel healthy.

"But not everyone is as lucky person, maybe now he does not have a heart attack but we do not know how later on," said deputy Secretary PB PAPDI (Indonesian Society of Internal Medicine Physician).

Some of the reasons often leveled by the smoker to continue to perform the activity. The reasons expressed by the smokers are usually:
  1. He considers himself in good health
  2. Not exposed to attack or heart disease even though he was active smoking
  3. Smoking or not smoking he would die too
"Risk factors for heart disease is multifactorial, and one of them are smokers. Some of the known risk factors can be modified or changed such as lifestyle and smoking, while other risk factors can not be modified such as gender and genetics," says Dr Sally.

Dr Sally also revealed there are some patients who had heart by pass surgery or heart attack but still smokes. Though the risk of heart attack again he will increase and the possibility of attacks that occur will be far more fatal.

Thus, to people who have never had a heart attack the first thing to do is to quit smoking, it also applies to people yabng ever stroke.

While Dr. Ari Fahrial H-Sham SpPD KGEH, MMB, FINASIM said cigarettes can also increase 2-3 times the risk of pancreatic cancer, increased 2-fold risk of colon cancer, increased risk of cancer of the esophagus (throat) and also make his ulcer disease not heal. Lung cancer is the dominant factor is caused by smoking.

"If it's addiction or the addiction is difficult to stop smoking, unless there is a consciousness of itself to quit smoking or it could be because've learned my lesson," he said.

There are several things you can do for someone to quit smoking that is not familiar with the term repeat cigarettes, keep all things related to smoking such as ashtrays or other items and a definitely must resist the temptation.

Heart attack (coronary heart disease) is now not only attacks the elderly but also young people. A heart attack is not sudden since before the attack appeared already there are signs that conveyed the body.

But these signs are often ignored. In fact, according to research from MIDAS in Milan, Italy in 2006, almost about 52 percent of patients with coronary heart disease did not experience chest pain or often referred to as silent ischemia.

Reporting from mayoclinic, Saturday (02/05/2011) coronary heart disease is a disease caused by narrowing or blockage of coronary arteries, the blood vessels that will drain blood to carry nutrients and oxygen needed to function of heart muscle to pump blood whole body.

Human heart pumps blood around the body secra constant at around 4-5 liters of blood per minute. Therefore, if blood flow is not smooth can cause damage to heart muscle that causes the heart pumps disorders (heart failure) and death.

Triggering a heart attack include excessive exercise, which spend a lot of emotional upset or other activities done to excess.

According to Dr M. Taufik Arifin Pohan, SpJP as quoted from his writings in detikHealth, at least there are 5 signs of a heart attack to watch out for:
  1. Stress (load seemed to be overwritten, pain, pinched and burned), which causes shortness of breath and choking on the neck.
  2. The pain may spread to the left arm, neck and back.
  3. The pain can last about 15-20 minutes and occur continuously.
  4. Arise in a cold sweat, weak body, heart palpitations and even fainting.
  5. The pain can be reduced when it is resting, but will gain weight if you're on the move.
Some ways you can do to prevent heart attacks, such as keeping cholesterol levels of blood fats, keeping blood pressure for control, stop smoking, avoid fatty foods, replenish fruit and vegetable consumption, exercise regularly, lose weight and reduce stress.

Liver, including the vital organs in the human body given the task is complicated by the continuity of all functions of the body. One of the deadly disease that attacks the organ is liver cancer.

Globally, liver cancer is a type of cancer in the third highest cause of death. In Indonesia alone, there were 13,238 cases of liver cancer according to research data GLOBOCAN 2008 with the death toll reached 12,825.

Most liver cancer is diagnosed in middle and advanced stage because at this early stage of development of this disease do not show typical symptoms. In general, symptoms of liver cancer are weight loss, stomach feels full, the hard mass on the right just below the rib cavity, skin and eyes become yellow green, unusual fatigue, or discomfort in abdominal upper right.

Statistics show that this disease affects more men with a ratio of 3:1 with a female.

According to Solomon Prof.dr.Ali, Sp.PD-KGEH, liver cancer can be detected through blood tests, ultrasound examination of CT scans, MRI, or biopsy.

"Ultrasound is a simple and inexpensive. If there are any bumps in the liver or an increase in new tumor marker such as a biopsy done further investigation," he said.

Treatment

Liver cancer treatment options depend on the type and stage of cancer. Ali explained that in general there are 3 options in the treatment of liver cancer cases, ie, resection or cutting of tissue with cancer, liver transplantation, as well as with radio frequency ablation.

"In the ablation technique, used radio frequency to produce vibrations that destroyed the tumor. Meanwhile, if the liver is still functioning properly, removal of the tumor to treatment options," he explained.

For patients in advanced stage, the selected treatment is palliative in nature or extend the patient's age. One of the palliative therapy that can be selected is the target of therapy.

Targeted therapy works by inhibiting a specific molecule that helps the growth and development of cancer. "The signals or blood vessels supplying the cancer growth will be inhibited so that cancer can not be food and eventually shrink," he said.

Sorafenib is the first oral medication to treat advanced stage liver cancer. In Indonesia, patients receiving these drugs could be extended an average age of 9-11 months.

Currently liver cancer patients who are less able to get sorafenib NexPAP program sponsored by Bayer HealthCare with Indonesia Cancer Foundation. Those who qualify medically and economically able to get treatment free of charge for 12 months.
